The Importance of Medication Management in the Home Health Setting

Several home health agencies are contributing to the serious and sometimes fatal risks as patient’s transition from hospitals to other settings. According to a Kaiser Health News analysis of federal inspection records, from January 2010 to July 2015, about 3,000 home health agencies recorded new patients’ meds.
An example of a medication management failure was when 66-year-old Joyce Oyler was given the wrong medication after a hospital visit for congestive heart failure in 2013. The result was Oyler’s death due to multiple organ failures caused by the medication.
The home health agency that cared for Oyler failed to catch the medication error. “This episodic care at different places at different times is not designed to keep the overall safety of the patient in mind,” Goldmann told Rau.
