PART-TIME WEEKEND  HOMECARE CODER/DOCUMENTATION REVIEWER

Responsible for properly identifying and sequencing diagnoses and review of OASIS with regard to appropriateness, completeness, and compliance with federal and state regulations and organization policy. Applies current coding guidelines, implements regulatory changes, obtains additional and/or clarification of documentation from physicians and clinical staff via clerical admin department to assure documentation supports diagnoses coded. The employee must be able to function with a high degree of independence and considerable discretion.

  • Conduct quality assurance documentation reviews of assigned records to include but not limited to:

o   OASIS for documentation inconsistencies and accuracy

o   Appropriate physician’s orders

o   Care planning as it relates to assessment and regulatory requirements

  • May be required to attend mandatory scheduled meetings/trainings despite flexible work schedule.

HOMECARE CODING TEAM LEADER

The Homecare Coding Team Leader leads a team of coders under the direction of the Coding Manager.  Activities will include but not limited to working with the coding manager to establish and maintaining company and agency workflow process related coding activities, ensuring productivity measures are being met, resolving disputes with agencies as needed, work with any external vendors to ensure standardization of processes and working with QA team to ensure quality standards are being met. The Team Leader must be able to function with a high degree of independence and considerable discretion

General Duties:

  • Carrying out and attaining the mission and the goals of the business
  • Direct, supervise and evaluate work activities of the coding team
  • Maintains quality service by enforcing quality and customer service standards
  • Establish work schedules and assignments for staff, according to workload, space and equipment availability
  • Maintain coding guidelines 
  • With the coding manager, coach, mentor, and develop staff, including overseeing new employee onboarding and providing career development planning and opportunities.
  • Empower employees to take responsibility for their jobs and goals. Delegate responsibility and expect accountability and regular feedback.
  • Lead employees using a performance review; and development process that provides an overall context and framework to encourage employee contribution and includes goal setting, feedback, and performance development planning.
  • Lead employees to meet the organization's expectations for productivity
  • Provide effective performance feedback through employee recognition and disciplinary action, with the assistance of Human Resources, when necessary.
  • Maintain transparent communication. Appropriately communicate organization information through staff meetings, one-on-one meetings, and appropriate email, IM, and regular interpersonal communication.
  • Responding to agency enquiries and complaints under the leadership of the coding manager.

 Qualifications:

  • Homecare Coding certified (HCS-D, HCS-O, or BCHH-C) or ability to obtain within a year.
  • Medical coding experience is preferred, but not required.
  • Previous Supervisory experience is preferred, but not required.

HEALTH INFORMATION MANAGEMENT SPECIALIST

The Health Information Management Specialist (HIM) is responsible for the activities related to the process of retrieving medical documentation from electronic medical record systems, for the purpose of coding and/or OASIS review. This role requires someone to function with a high degree of independence, dependability, data entry accuracy and maintain information confidentiality as per HIPAA practices.

Essential Duties and Responsibilities:
• Retrieve appropriate medical documentation pertaining to a specific assessment from various EMR systems. Upload the documentation into our proprietary web portal to create a complete file pertaining to a specific assessment to send to the coding department
• Contact the agency in the event of missing or incomplete documentation necessary for coding
• Following the completion of a coding and/or OASIS review, enter the ICD-10 codes pertaining to the assessment into the agencies EMR system
• Work with the agency to ensure that all assessments have been completed by end of month to allow for the agency to bill services to the appropriate party
• Generate various reports with assessment data and merge utilizing specialized software
• Manages problem files including incoming and rejection files
• Actively attends company meetings and educational sessions
• Maintains excellent customer service while engaging with clients

Qualifications:
• Must be able to function with a high degree of independence and considerable discretion.
• 3-5 years of related customer service experience
• Knowledge of Excel, Word, Outlook is required
• Information technology experience preferred
• Experience working with various electronic medical records is preferred
• Must be open to a flexible schedule
• Rare traveling may be required

CODING MANAGER

As a Coding Manager, you will manage outsource coding team of employees and contractors for ICD-10 coding and documentation review. Works collaboratively with other internal teams to improve clinical services and meet customer expectations.

Essential Duties and Responsibilities:
• Establishes professional and positive work environments by modeling respect and dignity toward all employees and acknowledging the value of individual’s effort and contributions.
• Provides feedback to supervisor to develop efficiencies.
• Works with the coding team to analyze needs, provides training along with resources.
• Provides oversight to team leads and coder/documentation reviewers.
• Orients new employee and contracted coding/documentation reviewers to their role and expectations. Assists with facilitating access and training on client's individual electronic medical records, specific workflow and HHCodePro.
• Proactively identifies coder/documentation reviewer concerns and requests assistance up-line as required. Coordinates with education and quality department as needed.
• Participates in strategy sessions, manages communication and information flow between team members.
• Coordinates outsourcing partnership with Project Manager.
• Assists in continual education of new and current outsourced coder/documentation reviewers.
• Collaborates with line management to ensure contractual obligations with partner agencies/projects are fulfilled.
• Acts as a customer liaison with accounts, builds relationships with key personnel at the agency and looks for opportunities to sell additional billable services.
• Proactively identifies and leads agency concerns and requests assistance up-line as required along with consulting with the education and quality department.
• Participates in the implementation process of a new agency.
• Develops individualized agency processes for coding and/or documentation review in a client's electronic medical record system in collaboration with the Team Lead.
• Conduct reviews of coding, OASIS and Plan of Care while providing recommendations following OASIS guidance, CMS rules/regulations and coding guidelines.
• Provide written and verbal feedback related to documentation and coding.
• Develop and maintain knowledge of EMR systems utilized by various agencies.
• Provides coverage for Sr. Coding Manager and Team Leads absence.

ADMISSIONS MANAGER - HOSPICE AGENCY

Job Title: Admissions Manager_Hospice Agency
Position – 3 month temporary assignment
Location –Northern California

Summary:

Responsible for coordinating all activities related to admission of patients to total care including providing information about organization services and maintaining optimal relationships with all referral sources in compliance with organization policies and procedures and applicable laws and regulations.

Supervisory Responsibilities:

  • Supervises non-exempt admissions team staff.
  • Provides clinical supervision to transdisciplinary team members of same discipline.
  • Provides administrative supervision only to team members of different disciplines.

Skills & Qualifications:

  • Strong working knowledge of nursing practices, preferably in a hospice care setting.  
  • General knowledge of management practices including relevant laws, regulations and standards, e.g. CHAP. 
  • Intermediate proficiency in MS Word. Basic proficiency in MS Excel. Basic to intermediate proficiency with a computerized clinical documentation system.

Education/Experience:

  • Minimum of two years of nursing, social work practice, or spiritual counseling experience in a home health/ hospice setting within the past five years.
  • Minimum of two years supervisory experience preferred, within the past seven years.

Certificates, Licenses, Registrations:

  • Current license as a registered professional nurse in the State of California, BSN preferred and/or Masters in Social Work, LCSW preferred or Masters in Divinity.
  • HPNA and/or PHN certification preferred.

Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
