Hospice, Palliative Care Providers Seek Advance Care Planning Best Practices

Advanced care planning has been a priority for hospices and palliative care providers for a long time, as they work to reach patients earlier in the illness to coordinate and plan for end-of-life.  “Research has shown that reaching patients sooner in the care continuum can be beneficial for providers and patients alike, as well as the families and caregivers who support them. Incentivizing advance care planning discussions to place a greater emphasis has been correlated to improved outcomes. Such discussions about patient’s end-of-life goals and wishes have been associated with improved access to quality of care and longer life expectancy.”

Discern Health and Coalition to Transform Care (C-TAC), worked to develop evidence-based best practices in advance care planning, also to create recommendations for hospice and palliative care providers on how to better incorporate goals-of-care conversations with staff, including ways to begin end-of-life decision-making process earlier.
