Participate in Survey on Communication Between Home Health and Physicians

Johns Hopkins University and NAHC are conducting a national survey of Home Health’s frontline nurses and therapists on the barriers on communication with physicians.
The paper survey, which will be mailed to your agency, is about five-to-seven pages long and should take about 10 minutes to complete. The survey should be filled out by nurses or therapists who provide direct patient care in the home. It is not intended for hospice.
The survey asks questions about the quality of communication with physicians, as well as what barriers they may encounter while trying to communicate with physicians about patient care.
If your agency receives the survey, Johns Hopkins and NAHC would greatly appreciate your input.
